🔍 We conducted a systematic review and meta-analysis of randomized controlled trials (including the recently published PRESTIGE-AF) to shed light on this challenging scenario. Across 4 RCTs including 653 patients, our findings highlight a complex risk-benefit trade-off:

Reduced risk of ischemic stroke (RR 0.23) 🔴 Increased risk of recurrent ICH (RR 3.60) ⚖️ No clear net clinical benefit (RR 0.72) 💔 Fewer MACE, but 💉 more major bleeding events

These results highlight the need for individualized clinical decisions, better risk stratification tools, and (most importantly) rigorously designed future trials to guide anticoagulation strategies in this high-risk population.
