What about #SABR / #SBRT to the primary tumour in LA-NSCLC? This study just presented at #ESTRO25 has promising survival with little toxicity rates ➡️ improvement in PFS ➡️ trend toward better OS

Dose metrics linked to bowel toxicity for rectal RT in RAPIDO: Amazing work by Max Tanaka from NKI Medium-to-high dose volumes for bowel bag (RTOG or Embrace) best predictors for both acute & late toxicity - V30Gy for LCRT, V20Gy for short course #ESTRO25 #radonc #medphys

Morning > Afternoon for IO? Evidence is mounting—retrospective and now prospective (210 pts, randomized, PFS HR 0.42 but no PD-L1 strat). Given IO’s long half-life, maybe only the timing of the first dose matters. #ASCO2025

On that note, I wonder if radiation oncology will ever have a community like Radiopaedia one where we can share full stacks of images, volumes, DVHs, and meaningful discussions, all driven by the same communal spirit. It can feel quite lonely in radiation oncology at times.

While I'm here, I'll just point out that RCTs of SABR don't show that radiation leads to more metastases...if anything the relationship is going the other way. Here's the data from Dan Gomez's oligomets RCT...

❓ESTRO & EORTC proposed a classification to standardize reirradiation. But does it predict outcomes or toxicity? 🫁☢️ We applied the classification in thoracic reirradiation for NSCLC. Here’s what we found. 🧵

I feel fortunate to have become a lung cancer specialist in the early 2000s and to have witnessed these exciting advances in both technology and biology-based diagnosis and treatments.
