CP case of the month 🔬: A man in his 70's presented with pancytopenia, weight loss, and lymphadenopathy. He was found to have elevated creatinine (1.7 mg/dL), elevated IgM (582 mg/dL), normal IgA, and slightly depressed IgG. Immunofixation showed the following:

⚡️ SPEP ➡️ SERUM Protein Electrophoresis. 🤯 Serum > 1,500 circulating proteins ⚠️ But NO Fibrinogen. 🤔 If you do Electrophoresis w PLASMA --> Fibrinogen can create a spike and can👀 like M Spike🦴 🧪 You can do IFE to verify, but that’s why you do SPEP not “PPEP”

⚡️ SPEP ➡️  SERUM Protein Electrophoresis.
