We find this specific strain carries no carbapenemase-encoding genes. Here, carbapenem resistance evolved from previous selection for ceftazidime resistance, with added potentiating evolution via ompK36 porin mutations.

However, the genetic loci that confer resistance are extremely costly to fitness, and sensitivity to carbapenems is rapidly restored in the absence of drug selection during evolution experiments.

This research has wide clinical implications, suggesting that combination therapy with ESBL inhibitors would be effective, and cautioning against phage therapy that utilises ompK channels for action against CRE.
