A year ago today I graduated with a Pgd in District Nursing. It’s bittersweet handing my notice in, to be taking a step away from DN and embarking on a new journey in the community in the new year, in a Trainee Advanced Clinical Practitioner role. Huge challenges ahead ✨

Today feels like the end of an era 🥲 The first photo was 7 years ago where I started working from this base, I have gained so many friendships. I feel so lucky to have been supported by an amazing team to get to where I am in my nursing career. DN will always have my heart. ♥️

To ensure timely discharge for patients in hospitals, more investment is needed for community services. Older people are particularly at risk of poorer outcomes if kept in hospital when medically fit - Jugdeep Dhesi

One of my proudest moments was receiving my QN award. Completed a shift yesterday with DN’s to support them through their never ending demand and capacity issues and absolutely loved it. Able to use my transferable skills from frailty whilst keeping up my DN clinical skills. 💙
