IME police investigations into fatal cases involving cyclists range from ok to abysmal. Coroners will often just adopt police conclusions because they do not have anything else. I would hope the police were closely cross examined.

On this day in 1896, Bridget Driscoll was walking in Crystal Palace Park, London, with her family, when she became the first pedestrian to be killed by a car. At the inquest into her death, the coroner said he hoped such a tragedy "would never happen again..."
