IMCrit In all seriousness, is there any other subspecialty where patients labeled as having a disease (asthma, copd, ohs, phtn, IPF, etc) so infrequently actually have it? Perhaps because pulm diagnostic tests are so bad?

Happy 70th Birthday to Ruby Bridges, who at the age of 6 desegregated the New Orleans public schools. She’s younger than 36 senators and only 7 years older than Barack Obama. The past isn’t past.

Me at the beginning of pregnancy: β€œof course I’m going to work until the due date! I’d be so bored otherwise” Me at 35 weeks: laying on the couch to catch by breath after doing one load of laundry with the baby kicking my diaphragm and wondering if it’s too early to go to sleep

Five years ago I had suicidal ideation due to repeated sleep deprivation. I was a resident working long hours, including 28-hour calls. Sleep deprivation is an independent risk factor for suicide. Any discussion about physician wellness is wholly inadequate if it excludes sleep.

ICU doc here. The most life-saving treatment developed for COVID is Dexamethasone. Dexamethasone is generic, cheap, widely available. Dexamethasone costs less than a dollar per day and saved millions of lives. It also completely destroys your argument.

Such an important inpatient lesson today! My intern knew something just β€œdidn’t fit” attributing a common dx to a patient - turns out it was a rare pulmonary complication! Remember you don’t know what you don’t know. It’s okay to tap an expert for help!

Great slide from Joshua Goldberg on symptoms that suggest imminent hemodynamic collapse in patients with PE. Symptoms may predict better than vital signs, labs and imaging. #PERT2024

But how do normal people get the recommended amount of prenatal care?! It’s so much! Esp if complicated. I have three visits a week (OB + NST/AFI) - no way I could do that if I didn’t work here and can run down quickly for care.

These drugs are lifesaving! They reverse serious chronic illnesses, make people eligible for procedures to improve or save their lives. Meds are meds - no need for shame

Anyone who thinks legal immigrants are the cause of rising housing prices need to look a bit into what private equity is doing to housing markets They are buying everything and making all single family homes rentals and driving up rents The billionaires are the enemy

Clean needle/syringe programs have actually been shown repeatedly to increase rates of cessation (stopping using) and increasing rates of people entering recovery So if you oppose harm reduction you are actually keeping more people using drugs and preventing people from stopping
