Challenging Cases: 84yo WM w/AS, did TAVR. Used single Proglide in LCFA. Proglide got stuck and foot plate mechanism would not disengage from Ca+. We deconstructed and safely removed from the arteriotomy. Fantastic tips & tricks, big credit to Dr Amy Daliman TAVR Bot

Recurring thought: In a world where other specialties are increasingly doing the “chip shot” image guided procedures and referring the “difficult” procedures to IR - at what point does it become justified to lobby for a difficulty modifier to the CPT? (Ie surgical Modifier 22)

Patient comes down for IVC filter. This is the initial venogram after some trouble getting wire into IVC. Then, a subsequent venogram is performed after a 30 second intervention (see next reply). What intervention was performed? SIR RFS

Not the most wild of pathologies but impressive example of a “nipple” or “daughter sac” sign on this AAA. RP blood but no extrav on pt presentation to ER w/back pain. Temporary tamponade. Angio shows blowing hole in the sac by time pt makes it to angio for emergent repair

One of the best experiences I had as a resident was going to DC to lobby for healthcare policy change. Thats where I realized how little those that decide healthcare policy know about healthcare practice This won’t stop street fentanyl- but it will take away a useful drug.
