The "pan-culture" mindset is out of control (doi.org/10.1093/ofid/o…). Instead of rushing to order BCx and broad-spectrum antibiotics, assess the patient first. Identify the source, establish a diagnosis with targeted tests, and avoid ordering every lab just to figure it out.

The routine ordering of ESR & CRP should stop.
