Blastic plasmacytoid dendritic cell neoplasm (BPDCN) in the bone marrow of an adult male: aspirate smear (left) showing blasts with cytoplasmic pseudopods; core biopsy (right) showing spindle-shaped blasts. Rare! Skin lesions are usually present, and MNDA negative (vs. AML).

Pathologists! This adult patient had elevated BUN & creatinine. Glomeruli look too cellular and too blue. Why? The capillaries are clogged with large B cells. This is intravascular large B-cell lymphoma; it can be tricky and subtle. #hemepath
