This paper is largely based upon legacy studies. Studies before the wide spread use of PCI in STEMI and non-STEMI patients. The rate of PCI was not reported. This paper is an academic exercise and should not change practice. PCI stratification is helpful.

A linear regression should not be used. A fractional polynomial mathematical model should be applied. Most biological systems are not linear instead they are S shaped curves.

When modeled with fractional polynomials rather than a simple cut off, post PCI creatinine shows a smooth non-linear relationship with long term MACE. This pattern suggests CA-AKI is often related to underlying health rather than an independent marker.

The evidence is weak for Class 1B since some of the studies had about 25 percent lost to follow up and missing data. A study with more than 20 percent lost to follow up is not informative nor reliable and should not be used by guideline committees.
