One of my fave parts of adv endo is the immediate impact on pt quality of life. This unfortunate patient had PEG tube removed at OSH w/ 2 months of persistent leakage. One quick endoscopy and zero leakage since 🤗 1⃣ APC to de-epithelialize tissue 2⃣ 12/6t OTSC for closure 🙌

EUS Guided GJ for GOO Key Steps: ✅Distend small bowel w/ fluid/contrast/meth blue ✅Identify loop w/ good runway for AXIOS ✅Advance AXIOS w cautery - commit, but not too forceful - let cautery do the work ✅Deploy & enjoy the sea of blue🤗 Pt eating for first time in weeks!😀

Additional optional step: After deploying the first flange, I advance a wire into the jejunum to act as an “in case of emergency” wire. Can have this preloaded in the AXIOS so it’s ready to go after entry into the small bowel.
