Always, always ask: Why is this person delirious? If they are showing agitation - always check for pain, retention, constipation, thirst, etc. #Delirium #PatientSafety #Geriatrics

πŸ‘‚ When speaking to a person describing delusional beliefs, listen without dismissing their experiences. Instead of immediately correcting them, say something calm & neutral like: πŸ’š β€œThat must be very frightening for you.” #delirium

..for me, an older woman taking more than 20mg amitriptyline may well have had something bad happen - loss of a child, abuse she doesn’t want to talk about. We need to treat that situation w respect+care. Listen, explain, try to reduce, but sometimes just leave it on the chart.

Our national study found that for every one hour with patients, UK resident doctors spend four hours on admin. In The BMJ today, we break down why this isn’t just an inconvenience, it’s a safety risk.Β  1) Fragmented EHRs β†’ endless copy-paste + re-entering data 2) Trainees

Words matter BGS (& others) rightly choose to use hospital at home over β€˜virtual ward’ with good reason Use of the word β€˜virtual’ can imply patients will only receive hands off remote care when we know people with real needs often require care to be delivered fact to face πŸ‘‡

Emergency Department (ED) staff: screen ALL patients over 65 for delirium. Emergency departments are where it's most missed and most dangerous. Make it as routine as checking blood pressure.

This is a key message to nhs managers and ICBs. Older people live with complex needs, multimorbidity, often disability & there is large overlap with frailty as a LTC. To get integrated care & neighbourhood health right, focus on the evidence and key principles within CGA

Good morning everyone. I’m at the #BGSconf this morning talking about delirium. Caught Claire Swarbrick’s excellent talk on the impact of delirium and the results of Snap3. Delirium increases length of stay, mortality and all complications.
