A 63-year-old female. Peak gradient of 76 mmHg in the LVOT, with moderate to severe mitral valve regurgitation due to SAM. We performed a right anterior thoracotomy, which provided excellent visualization of the LVOT, a key reason we routinely use this approach. Oleksandr Babliak
