In the finale level 3Heaven1, once the player climbs the ladder at the start, for less than a single frame the words "Forgive me" in white text pop up. This is both a hint at the reality of the situation, David's guilt, and the answer to the 4th ending code.

It's a huge spoiler, but would you guys like to know about the secret weapon only accessible via getting all 4 endings?

Newly discovered secret: In the intro Nightmare scare with the psych ward, you can see a hanging bag-headed man. You can only see this on framerates 60+ because on lower framerates (-40) it's too fast.
