Septic sacroiliitis in infants/ children is a much more rare etiology of hip pain/ fever and refusal to weight than septic hip infection or osteomyelitis of the proximal femur. It can be managed medically in most cases without need for surgical debridement. MRI is main dx method

For femur fx in peds patients with open greater trochanteric apophysis, I prefer the lateral entry nail (rather than regular troch entry nail from tip) to further protect the blood supply of the femoral head. Nail insertion is not technically easy, however, it still can be done.

For hexapod, before removing it, I will remove one strut in the office, ask the patient to walk, then come back next week. If not having major discomfort, I will remove another one and book for frame removal the following week. If having pain, strut is applied again.

"Positive buttress position" (distal femoral neck fragment is medial to the lower-medial edge of the proximal fracture fragment) as described by Gotfried in 2013 gives stability to fracture neck femur. These can be fixed by percutaneous screws with good results.

Pediatric traumatic bone defect can be treated by bone grafting or bone transport. Rarely, I will use "allograft". It carries higher incidence of infection, but can avoid lengthy recovery and ExFix inconvenience. Good healing potential in kids allows the usage of this technique

In the last year, I had 3 patients with fracture osteochondroma stalk, causing sudden pain. This was not common finding for me before. I think because kids are more aggressive in sport activity, this may be seen more frequent.

Percutaneous Gigli saw osteotomy can still be done even with the presence of IM nail. It will require changing the direction of wire pulling 2 times. First time, pulling from the medial and lateral side, then both from medial and then both from lateral.

The clinical significance of positive Barlow test youtu.be/lqtLIhNnJUw?si……… had been questioned in recent literature. In 2000, AAP recommended "orthopedic referral" for patients with positive Barlow,this was changed to "sequential follow-up examinations" in 2016 recommendation.

Pts with genu varum (especially higher degrees) and medial compartment OA, I prefer to do the HTO using ring fixator. Exact correction can be obtained. Osteotomy distal to the tibial tubercle, no patella baja. Distal tibia should be translated as osteotomy is distal to the CORA

Acute deformity correction with internal lengthening using IM nail requires meticulous pre operative planning to ensure proper correction of the deformity. Fixator assisted osteotomy and nailing can help in mitigating some of the risks of inadequate corrective osteotomy.

Hip flexion contracture is not something you will observe in your patients when they are lying down flat on bed as it will be masked by lumbar hyper lordosis. Hyper flexing the contralateral side will obliterate the hyper lordosis and manifest the deformity (Thomas test)
