Cool idea in general, but still waiting for a trial proving that stopping meds folks are tolerating improves outcomes. We should stop meds that don't offer benefit regardless, but the speed/lengths we go to should depend on magnitude of risk reduction offered by deprescribing.

Is there a service that offers patients a doctor/nurse to accompany them during outpatient visits? Like pay a small fee get an expert to help+make sure you're heard? The care my family gets for even simple concerns when I'm there vs when I'm not is staggeringly different.

We need an index that describes how often your paper is cited vs how often it is read. Said index for "Crossing the Quality Chasm" would be enormous. If you had to prove you read a paper before citing it, 92% of the background sections in the medical literature would be gone.
