Tip of the day 💡: Bilateral symmetric diffusion restriction in the peri-rolandic cortices can be a strong feature of POLG-related disorders, within the spectrum of mitochondrial depletion syndromes. It was the only clue in this case. No oxymorons there, just a helpful tip 😉.

Tip of the day 💡: Before you call such signal changes nonspecific (or even ganglionic perivascular spaces), think again. They are bilateral and right in the middle of the putamen. Final diagnosis: a mitochondrial disorder, namely IARS2.

Beware of low antibodies as a complication of b-cell depleters. This is a case report of a 34 year old man on ocrevus for MS who developed catatonia and fever. MRI showed symmetrical thalamic lesions. He had enterovirus encephalitis. IgG was 4.92 g/L neurology.org/doi/10.1212/WN…

Beer drinkers odd ratios for Parkinson’s Disease compared to non-beer drinkers: 0.79 for <1 drink/day, 0.73 for 1–1.99 drinks/day, and 0.86 for 2 or more drinks/day. Is it uric acid? Is it the relaxing social time? Does yeast survive and change the gut?
