People with PSC have a unique pattern of bile acid synthesis

They sampled bile using ERCP among controls (no biliary disease), other cholestasis, and early PSC

And they discovered something important about psc pathophysiology

Everybody talks about using the biomarker PEth to detect alcohol intake. But it's costly: $100-300.

Instead, start doing a good motivational interviewing with the patient following the RULE method. It takes time and empathy, but it's much cheaper.

Naltrexone in AUD and liver disease:

🥇Should be 1st line

🔹Start: 50 mg daily, also available as monthly subcutaneous

🔹Use freely in Child’s A/B, ⚠️ in C but benefits will probably outweigh the theoretical risks as 🍸is ☠️for liver
