Practicing knot tying is underemphasized in the training of those in non-surgical specialties. Many years of observation has shown that any discomfort in tying knots will add *significantly* to the overall cognitive burden of doing a procedure. The goal is to get rid of this.

🧵 "What really determines tissue perfusion?" – and why most explanations get it wrong. Let’s sort out MAP, CVP, CCP, autoregulation, vasopressors, and the flow that actually reaches your organs. 👇

🧵 What is Critical Closing Pressure — and why does it matter for perfusion? A thread to clear up one of the most misused and misunderstood ideas in circulatory physiology. 👇

Correct answer: unlikely free fluid. Seminal vesicles are a potential pitfall (confused with free fluid) when interpreting transverse #POCUS images of the male pelvis during #FAST exam. Fanning the probe superiorly brings peritoneal space into view. Recommend scanning in long

Did you ever place an arterial catheter and just the next day it stopped functioning? I thought it was due to my poor technique but it seems that we should pay greater attention to the catheter material Note: this study was funded by Vygon France

1/ 💉 Resuscitation fluids – life-saving in true hypovolaemia. Give them for the wrong reason and you harm patients. We’ve covered maintenance & replacement fluids — now the trilogy ends with resuscitation. Here’s how to get it right. #MedX

1/ Shock isn’t “give fluids, then pressors, then inotropes.” That recipe misses the physiology. Here’s how to manage shock properly: 🧵 #MedX #haemodynamics

8/ Fluid responsiveness ≠ hypovolaemia. It just means Pms-responsive. The neglected twin is vasopressor responsiveness — venoconstriction ↑stressed volume and Pms.
