Antibiotic Steward Bassam Ghanem 🅱️C🆔🅿️🌟 Stomach acid evolved over millions of years. We use ATP to make the acid. While there are issues with overproduction, it makes no sense to me to expose 25-40% of the population to these potent drugs, often long term.

Yes. Writing is not a second thing that happens after thinking. The act of writing is an act of thinking. Writing *is* thinking. Students, academics, and anyone else who outsources their writing to LLMs will find their screens full of words and their minds emptied of thought.

"However, the concept of static/cidal antibiotics is neither complex nor context-dependent;it is rather a myth. Regrettably, this dogma continues to be raised in modern literature despite two fundamental flaws" Honored to contribute to this important letter led by 🌟 Brad Spellberg

It's out on #AAC (ASM): a concise but incisive letter on the lingering false myth of "bacteriostatic" #antibiotics as inferior to "cidal" ones. What an honour to be among authors in a group led by Brad Spellberg. #IDXposts rita murri journals.asm.org/doi/10.1128/aa…

The Static/Cidal letter to the editor at AAC made Google News! First time I’ve ever seen that happen to a letter to the editor—must be a ton of people reading it!

Studies were of low to moderate quality and their results indicated a high probability that shorter antibiotic prophylaxis durations (including ZERO days) were noninferior to longer durations for all-cause mortality, but not for early rebleed or bacterial infections.

Another collaboration led by PhD student (and core internal medicine resident) Connor Prosty I think guidelines should be less explicit on duration (I think 3 days maximum) and I welcome a large modern RCT in this space. Emily McDonald Brad Spellberg

One cannot look at the actual meta-analysis plots (below) & be at all worried that holding abx increased mortality. If anything, it looks like shorter or no abx improved survival over giving the abx. The trend becomes even stronger for not giving abx for RCTs after 2004.
