Naming frailty but not grading it risks ageism. Frailty is heterogeneous; lumping people living with mild frailty and those with severe frailty (and enacting policies for "the frail") fails on its face. The goal is not rationing, but reducing risk that accords with preferences.

1/9 Was asked good questions re: the Clinical Frailty Scale, so this Tweetorial. Case 1: 84 year old woman; hypertensive, hypothyroid (meds for each) IADLs OK. Walks 5-7 km/day + 15km/week to visit her sister. "Would you still classify her as CFS =3 due to her chronic diseases?"

The ONLY way more functionally dependent people get home is if home care is available. Increase ‘capacity’ yes, but NOT in the hospital.

Great question. Used in acute care the CFS is meant to describe the patient’s state two weeks before they became ill. A defined baseline facilitates care planning: is recovery to that state likely? How navigable is return home if baseline achieved? How does carer feel about it?
