After a severe TBI, the risk of unfavorable 1-yr outcome increases by 1% with each additional day without command-following . This was observed in two independent studies, together including > 9,000 patients. We dub this the "1% rule". manuscript: tinyurl.com/yvs9s7xu 2/4

Time to command-following is generally not a very good predictor, only weakly discriminating between 1-yr outcomes (AUC 0.6). A 90% likelihood of unfavorable outcome is reached only after MORE THAN 40 DAYS without command-following-- an extremely rare occurrence. 3/4

In a family meeting, when one is emotional, cognition doesn't work. Before moving the conversation forward, you need to address their emotion.
