個人的に考えるTAILORED-AF試験のポイントです。 1) AI base (Black Box) 2) AF再発は減少/AT再発は増加 (AF/AT再発ではp=0.09) 3) 6ヶ月以上持続するAFではAF再発も、AF/AT再発も減少（真の持続性AFで有効） 4) PVI-alone群との非再発率の差は21%：TAILORED群の中には不要な焼灼を受けた患者もいる

HRSのLate breakingの1つ。S-ICD+対応するmodular LPM植え込み後のATP therapyのfeasibility+efficacyの検討。 97.4%でstable pacing captureが可能で、ATP success 61.3%とTV-ICDと遜色ない結果。 日本での使用は少し先でしょうが、TV-ICD vs. S-ICDの患者選択の議論は白熱しそうです。 ＃HRS2024

JHRS2024教育講演 PVC起源同定で、I誘導のpolarityがどこで変わるか？を考察しました。 ・RV中隔：I誘導陽性 ・LV中隔：I誘導陽性 ・後乳頭筋：陰性陽性混在 なので、下端は乳頭筋あたり 上端は ・RVOT/LVOT：陰性陽性混在 ⇒2者を結んだラインと推測。 文献ご存じの方は教えて下さい。 ＃JHRS2024

I誘導続き I誘導はRVOT/LVOTの鑑別には使用しにくいですが、OT起源のPVC ablation時には必ず見ています。 それは「Pacemap時にI誘導だけ極端に点数が異なることが多い」からです（文献ないです）。 「波高変化」よりも「極性変化」の方が、よりscoreに反映されやすいと解釈しています。 ＃JHRS2024

ESC2024より不整脈の話題。Withdraw AF 試験は少規模(N=60)ながら秀逸な研究でした。 頻脈誘発性心筋症に代表される、洞調律回帰後に心機能が正常化した患者を対象に、心不全至適薬物療法を継続するか、withdrawするかのRCT。 結果は安全にwithdrawできるとのこと。論文化が待たれます。 #ESCCongress

Excellent presentation and discussion! In the SUPPRESS-AF trial, LVA ablation in addition to PVI did not prove significant efficacy in patients with persistent AF. However, potential efficacy was suggested in patients with advanced LA remodeling. #ESCCongress #OCVC 増田正晴

This Wednesday, we have just launched PFA using PulseSelect®and performed 5 cases until today!🎉 In our institution, where general anesthesia is not used, coughing and body movement are critical issues. 😖 We are in the process of exploring appropriate sedation methods.🧭 #EPeeps
