Lets go step by step! There are occlusions of the right coronary (red) and circumflex arteries (yellow).

The "mass" in the CS is a pseudoaneurysm ( neck narrower than the diameter of the aneurysm) of the inferior and lateral basal segments of the LV.

Due to these findings, we performed a late enhancement acquisition, which demonstrated the presence of a thrombus (blue) within the pseudoaneurysm and enhancement of the pericardium/myocardium that contained it (yellow).

We also assessed myocardial viability, identifying transmural enhancement in the lower basal segments and areas of subendocardial enhancement (many greater than 50%; yellow) in the basal lateral segments (red).

Our conclusion was: Myocardial infartion complicated with a LV pseudoaneurysm. CAD-RADS 5 / P4 / HR / E. Remember cardiac imaging is dynamic, always be ready to modify our protocols and get the best of each scan. Have a great weekend!!
