Unequivocal evidence that pulmonary functional impairment even in moderate COVID-19 is predominantly vascular. Alveolar function is normal. Gas exchange against the pulmonary capillary bed is impaired. It is a dead space issue even before thrombosis begins.

Have reviewed a lot of papers on clots in severe COVID-19 in last weeks & most ignored immunothrombosis as cause of segmental/subsegmental changes on CTPA & counted them as “PE” It’s time to for all to acknowledge this phenomenon & move on!

Pleased to share our letter in The Lancet Respiratory Medicine. We propose that pulmonary infarction (in COVID-19 and other pulmonary vaso-occlusive disorders) occurs primarily due to congestion and oxidative stress rather than due to hypoxia. thelancet.com/journals/lanre… @NUHS
