A new study from Dr Chris Bannon, Professor Julian Walters, and colleagues at the University of Cambridge has identified a key gut hormone — INSL5 (Insulin-Like Peptide 5) — that could explain why diarrhoea is so severe in BAD, a condition often misdiagnosed as IBS-D.

🔍 What Did They Discover? • INSL5 spikes when bile acids reach the colon — triggering a faster and more urgent need to go. • People with BAD had significantly higher INSL5 levels (often above 100 pg/mL), compared to healthy individuals.

• Around 40% of IBS-D cases also showed raised INSL5 and responded well to ondansetron, a drug that blocks INSL5’s effect in the gut.

🩺 Why This Matters • This discovery could lead to a new blood test for diagnosing BAD by measuring INSL5 levels. • New clinical trials are planned to test ondansetron’s effectiveness specifically for BAD. Recruitment is expected to begin in late 2025 or early 2026.
