A #FitnessToPractise client of my team died by suicide in March 2022, following a serious error in his case by the HCPC. It is now November 2022 and the HCPC has still not concluded its lessons learned review (much less shared it). I am both so sad and utterly disgusted.

Some takeaways after my conversation with a hospice nurse of 12 years based on her experience: 1) Many people will not pass away unless they feel ready and this can sometimes mean getting their hair washed, being shaved, or having a bowel movement.
