So even if inflow is okay if outflow pressure is high then perfusion decreases. As perfusion decreases, you have more end organ damage which triggers the inflammatory bomb of shock.

Tautening, traveling of perclose knot is critical to achieving hemostasis with minimal to no bleeding. When using MCS in shock, these same principles are needed to limit risk of complications. Hence in my opinion almost every case should be pre-perclosed if possible.
