Another day, another woman diagnosed with severe endometriosis after years of being dismissed and subjected to substandard scans. WE NEED TO NORMALISE INVESTIGATING PAINFUL PERIODS. WE NEED TO NORMALISE HIGH QUALITY GYNAECOLOGICAL ULTRASOUND.

Expectant mx is well-established in a few centres but is not practised nearly often/widely enough and I suspect that a substantial part of the reason is fear. This is not unreasonable - we’re taught at medical school that ectopic pregnancies kill. And sadly, they still do 2/20

Thankfully, medical advances mean that deaths from tubal ectopic pregnancies are increasingly rare, at least in the UK. Ultrasound in particular has not only transformed how we diagnose ectopics but also our perception of how dangerous they are 3/20

Until 1884 tubal ectopics were only diagnosed at autopsy and so were thought to be always fatal until Scottish surgeon Robert Tait reported the first successful surgical treatment of ectopic pregnancy by emergency salpingectomy. Ectopics were no longer universally fatal 4/20

With the advent of ultrasound, we were able to diagnose ectopics in stable patients but on early ultrasound the only ectopics we could see were large or ‘live’ both of which needed surgery. We had moved from ‘always fatal’ to ‘always needing surgery’ 5/20
