qSOFA 2+ does *not* mean sepsis is likely present. It identifies patients “at high risk of a poor outcome” from infection. It’s a risk stratifier, not a diagnostic tool. Know how your clinical scores were derived.

Using the Neutrophil-to-Lymphocyte Ratio as a “free upgrade to the CBC” (h/t 𝙟𝙤𝙨𝙝 𝙛𝙖𝙧𝙠𝙖𝙨 💊) helped me to diagnose aortic dissection in a young patient recently. If you’re discharging a patient with a NLR >10, it’s worth a second look.
