Some of the most dangerous catch phrases used in the hospital are "chronic", "at their baseline", "that's normal for them" a it anchors to: permanent, diagnosis/plan is correct & people aren't as bothered by it happened twice on service took me a min to realize it wasn't true

if you suspect C diff and the GDH/PCRs are negative multiple times you should maybe reconsider the diagnosis. If you still empirically treat CDI multiple times to no benefit - you might just want to consider other etologies. Or not. “ID referral: recurrent tx refractory c diff”

to my continental european colleagues have you all seen or heard of borrelial lymphocytomas w/ delayed diagnosis >1 year(s) after onset but w/ persistent lesion? most of the cases Ive been able to find have been diagnosed <2mo of onset

first, antibiotics correctly narrowed to azithro after diagnosis made (good!) but because of QT prolongation concern it was switched to an inferior antibiotic (wtf bad!) lesson: harms > benefits if prioritize preventing rare adverse effect over treating active severe illness
