IMCrit In all seriousness, is there any other subspecialty where patients labeled as having a disease (asthma, copd, ohs, phtn, IPF, etc) so infrequently actually have it? Perhaps because pulm diagnostic tests are so bad?

Five years ago I had suicidal ideation due to repeated sleep deprivation. I was a resident working long hours, including 28-hour calls. Sleep deprivation is an independent risk factor for suicide. Any discussion about physician wellness is wholly inadequate if it excludes sleep.

ICU doc here. The most life-saving treatment developed for COVID is Dexamethasone. Dexamethasone is generic, cheap, widely available. Dexamethasone costs less than a dollar per day and saved millions of lives. It also completely destroys your argument.

Such an important inpatient lesson today! My intern knew something just “didn’t fit” attributing a common dx to a patient - turns out it was a rare pulmonary complication! Remember you don’t know what you don’t know. It’s okay to tap an expert for help!

But how do normal people get the recommended amount of prenatal care?! It’s so much! Esp if complicated. I have three visits a week (OB + NST/AFI) - no way I could do that if I didn’t work here and can run down quickly for care.
