Bob Wachter In our outpatient mAb/anti-viral clinic in San Diego we’ve seen this 5 times (~2% of Paxlovid pts thus far). Definitely real, but seems mild, with more implications for isolation/infection control than real morbidity. Agree shouldn’t distract from otherwise great efficacy.

I developed Covid symptoms and tested positive Wed By Sat, I felt fantastic Today, I feel like death. If I were eligible for Paxlovid, would have finished yesterday. Be cautious of rebound symptom data— reporting bias is strong, my N=1 tells me it happens w/o treatment too :)

SF folks: it's now a big-time surge. No longer just cases (~500/d reported, so true # >2K, or ~250/100K/d). Also major uptick in hospitalizations: 98 in SF (vs 18 six wks ago). UCSF Health 41 pts in hospital, twice April #. If you're trying to stay well, time to up your game.

NNT = 15.3 [10.9 - infinite]. Same ballpark as we've seen. Remains possible there are different consequences wrt long covid, etc, but encouraging that tx is worth prescribing even in lower risk pops. (Works out to ~67 thousand ER visits saved per million doses to vax folks.)

Very interesting if true. One meter is closer than I get to most strangers in a park unless it's very crowded, but would certainly have import for things like music festivals, sports events. But if true, shouldn't we have already seen this signature eg at football games?
