#CytoABCs! J is for JC virus, polyoma virus (BK is the other more common one) causing viral cytopathic effect in urine Termed Decoy cells due to mimicking HG urothelial CA: ➡️Enlarged nuclei ➡️Clumpy chromatin ➡️Glassy nuclear inclusions ➡️Elongated comet-tail cytoplasm

Don't let the intranuclear inclusions fool you! Medullary carcinoma can have them too 😮 (this case fooled me though😐- check out the powdery chromatin and nuclear groves..but this is a recurrence in a proven case of medullary carcinoma in a 12 year old).

#CytoABCs! U is for Umbrella cell, covering top of transitional/urothelial epithelium! Large, stretched, w/ polygonal borders. Often multinucleated (can have lots of nuclei and look scary!), w/ stippled blue-purple cytoplasm (contrast w/ squamous cytoplasm), can have vacuoles.

#CytoABCs! V is for Vacuoles, large or small, seen in cytoplasm of various cells. While vacuoles are often associated with adenocarcinomas (A), you can see vacuoles in everything from the umbrella cells of U to benign mesothelial cells and histiocytes (both present in B).

Muscle disuse atrophy: These cells are benign atrophic skeletal muscle cells. The myocytes shrink as they lose the sarcoplasm but the nuclei remain intact. As such, they can sometimes mimic neoplastic process.

The real reason they discourage MRIs during pregnancy is because then people would realise they’re incubating nightmare demons and would be rightfully terrified
