We at Confluent Care believe that strong communities and relationships are key to helping patients find wellbeing in our healthcare system.

We're a student start up working to provide new tech to community health workers who visit homes for health coaching.

Today we are at the Spectrum Health Community Health Worker Conference in Grand Rapids to learn from the CHWs on the front lines.

1. In the last year there has been 28% growth of CHWs in United States!

2. Changing reimbursements from fee-for-service to value-based care has driven a lot of this growth.

3. During home visits, CHWs can recognize the determinants of health that doctors miss 9 times out of 10.
