“Can they live how they used to?” Is so important and I think people forget this

Assessed a patient straight from the ambulance as part of the Assessment Discharge and Transfer team today, following investigations they were safely discharged home with a mobility aid and referral to community services. They spent than 8 hours spent in the department😀 #D2RA

A rewarding day working on the Falls and Frailty Car. 3/3 people who were referred were kept at home with equipment, advice and signposting. 3 people very happy to avoid an unnecessary trip to hospital during these unusual times! RBH THERAPY TEAM #rightcarerightplacerighttime
