Happy September!! Ultrasound case for today. Called to code blue, ETT placed with DL, difficult view. No ETCO2 after placement but CPR in progress. Auscultation difficult with compressions resumed immediately after ETT placed. US used to assess tube position.

This is the “double barrell” sign with air noted in the trachea in the left and an ETT noted in the esophagus on the right. US can be useful in these situations but lack of ETCO2, no lung sounds, and likely air rush heard when listening to gastric area should lead to diagnosis
