It may come to you as a surprise, but I really don't care what a skin test or blood test is for a given food which you can tolerate if full servings.

Sincerely,

Friday dad-joke submitted by my LO

Why was the cookie so sad?
Because her father was a wafer (pronounced: away for) so long.

Don’t lump all oral immunotherapy together.

When looking at challenge defined outcomes, may not correlate with patient defined outcomes

Remission has been shown to improve QOL in patients that can achieve this!

Desensitization therapies mainly benefit more sensitive patients … protect against accidental peanut contamination. The patients with higher thresholds benefit from OIT that induces REMISSION! Again, this speaks to phenotypes of food allergy and the therapies we offer.

We are still working on the definitions of outcomes of oral immunotherapy. It’s not all the same AND we don’t even have our terminology sorted out - yet! Keyword here: YET!

Changing the course of allergic rhinitis- Thousands treated in one practice with sublingual immunotherapy changing the course of this disease and likely asthma prevention (need bigger trials to show this effect). Dr. Douglas Mack

Number needed to treat, let’s consider this in our discussions for allergic rhinitis.

NNT for statins = 40!
NNT for antibiotics = ~10
NNT for immunotherapy = 4! WOW 🤯
This is disease modifying therapy that has huge power!

The grade of evidence for the environmental control measures for allergic rhinitis we routinely talk about is actually quite LOW!
