For so many musculoskeletal conditions, to me, it makes so much more sense to bypass primary care and surgical specialties and simply go right to a physical therapist. Does not seem to be a very controversial opinion any longer to me.

posted many over yrs but, don't think we have answers. For me, at least 60% of subchondral head insuff tx's occur shortly s/p steroid inj. Whether correlative or cause no idea. But a common history is "acutely worse pain after steroid injection"...MR...fx🤷‍♂️

I think one of my biggest complaints with some orthopedic practices is their ridiculous use of steroid injections as if we have data to support so many of these injections around joints, etc. Where and why did this ever start? Is it a reimbursement situation?

Great take on why the CFT trial is not the gospel. With all due respect, CFT is just another narrative based paradigm. It’s a nice “psychosocial informed” intervention with outcomes that are similar to any other narrative led paradigm (biomedical etc). Stop idolizing narratives
