A 16 yr old/male with sickle cells anemia presents with shortness of breath and fever 😭 Hgb: 65 g/L WBC: 3.4 x10^9/L Plt: 550 x10^9/L LDH: 250 U/L Bone marrow aspirate 👇🏽 What do you see? Ddx? Management?

Gelatinous transformation of bone marrow = a jelly-like matrix replacing fat and hematopoietic cells. Seen in HIV, malnutrition, chronic illness, and cachexia. Adipocytes vanish, marrow thins out, and mucopolysaccharides fill the gaps. A marrow literally starving for energy.
