The findings in this study include identification of transcripts in IgA vasculitis skin biopsies that may predict development of subsequent nephritis, namely S100A8/9 (calprotectin), IL9, and transcripts encoding activating Killer cell immunoglobulin-like receptor (KIR) proteins.

Second, we demonstrate that compared with IgA nephropathy or IgA vasculitis, the immune transcriptome of IgA dominant infection related glomerulonephritis is enriched for genes involved in the innate immune response to bacterial infection.

Vacuoles are present in myeloid and erythroid precursors in this bone marrow aspirate performed for cytopenias. The Ddx is broad; this is a case of VEXAS (vacuoles, E1-ubiquitin-activating enzyme, X-linked, autoinflammatory, somatic) syndrome.

VEXAS is a hematologic and rheumatologic syndrome that is characterized molecularly by somatic mutations in the UBA1 gene. Patients present with a combination of autoimmune/inflammatory findings and cytopenias. pubmed.ncbi.nlm.nih.gov/33108101/
