💗CKD and atrial fibrillation
💊DOACS seem a better option than VKAs, however, 2 trials comparing them failed to recruit the necessary number of pts

DOACs are still off-label at eGFR<30 in 🇪🇺

❤️‍🩹How about left atrial appendage closure? pubmed.ncbi.nlm.nih.gov/38656567/

DASH trial- post hoc analysis
⬇️sodium diet
📍Minimal impact on glom hemodynamics
📍Associated with lower SBP
🧂Significant reduction in eGFR vs high-sodium DASH diet
🍌Effect on eGFR decline partly mediated by changes in DBP and potassium load

⭐️Discussion⭐️
T3a: ApoE is a protein involved in the metabolism of fats.
It has been implicated in:
🧠 Alzheimer’s disease
🫀 CVD
👁️ Macular degeneration

In the 🫘 has been detected in fibrillary, immunotactoid GN, monoclonal Ig deposition disease (and, now in DDD!)

T0f: C3G is mainly caused by dysregulation in the complement cascade, this can be from genetic 🧬 and/or acquired 🧪 factors.
More than 90% of C3G cases are caused by dysregulation in the alternative complement cascade, while 10% are in the classical/lectin pathways.

Sayali Thakare Nephrology Journal Club Anvitha Rangan This is one of my slides from a C3G talk. The lipodystrophy occurs due to nephritic factors leading to uncontrolled complement activation breaking down fat cells.
Unlike most other complement proteins which are made in the liver, >90% Factor D is made in adipose tissue.

T0g: Genetic 🧬 factors
♦️Most common mutations  ➡️ C3 convertase & Factor H gene

Acquired 🧪 factors
♦️Most common autoantibodies  ➡️ C3 & C5 nephritic factor

♦️Other etiologies: monoclonal gammopathy in older adults

T0h: EM 🔬 is required to distinguish between C3GN & DDD.

DDD  ➡️ extremely electron dense “sausage-shaped” deposits 🌭.  These are typically present along the glomerular, tubular basement membranes, Bowman’s capsule & mesangium.
C3GN  ➡️ not-dense/ amorphous deposits.

T0c: C3G is subclassified in: C3GN and DDD.

Both share many overlapping features making it a diagnostic challenge.

Check out this nice overview❗️
