The variety of options and patients is why, in my opinion, RCTs are unlikely to answer many of the pending therapeutic questions in autoimmune neurology.

We sought to identify the minimum prednisone dose and duration required to delay relapses. In our study of 109 children and adults with MOGAD, we found that 12.5 mg/day of prednisone in adults (0.16 mg/kg/day in kids) for >/3 months reduced the risk of relapse by 88% !!! 🧠👀

There is emerging data that early relapses in MOGAD increase the risk of a chronically relapsing disease course. Corticosteroids are highly effective treatment in MOGAD, but high dose long term exposure to steroids has an unacceptable side effect profile for our patients.

Exciting study from the group of my colleague Sarosh Irani AutoImmune Neurology Group!
They show that numerous biomarkers of neurodegenerative and neuroinflammatory diseases are detected in the brain-draining cervical lymph nodes, and in many cases in higher concentrations than blood!!! 🤩🤯😍👍🏻

A great paper led by SKWright
which emphasises the elegance of patient derived monoclonal antibodies in being able to ask clean, focused, scientific questions.

They are sufficient to cause frequent seizures in animals!
