An indoor mask mandate that only covers people who are unvaccinated and relies on an honor system is... Not a mask mandate. Please everybody please keep wearing your masks for just a little longer in supermarkets, stores, salons etc. Let's keep NYC's mask solidarity going.

Recalling a kiddo I saw weeks ago who presented with DKA. I ran through the 5 I's of DKA triggers in my head, ready to figure out his why. Houseless & with no fridge, his insulin was left to go bad in the car. No fancy ddx - just a reminder that housing is healthcare.

1k+ cases now being reported daily in NYC. When you're in a supermarket, store, subway car, movie theater etc, assume someone there may have the virus (and—especially if you're unvax'd—it may be you). Please wear you mask in these places. And if you still haven't—get vax'd NOW.

Tomorrow is the 100 year anniversary of the discovery of insulin. In 1921, insulin’s discoverers sold its patent for $1 so insulin would be affordable to all. Today, a single vial of insulin that lasts me about a week is priced at $300+, only in the United States.

Update on cluster of Legionnaires cases in Central Harlem: now 12 patients identified, all hospitalized. This is likely due to a contaminated cooling tower. All towers in area are being inspected/cleaned. In meantime, anyone with flu-like symptoms in area: seek immediate care.

On my EMS rotation, we responded to a call about a dying child. Blood sugar was >600, kid looked awful. Mom was sobbing, saying she couldn't afford insulin. I'll never forget what the EMT (later) told me: "You can't be poor AND have diabetes in this country...u gotta pick one."
