❓ What do you make of a ➕ 3D-CAM with only:

🔹 1 day off on day of the week (q2)
🔹 1 error on reverse digit span (q4/q5)
🔹 Subjective confusion (q8)

I wouldn't diagnose this as .

I really do like the 3D-CAM for its reliability, but it's overly sensitive at times.

Interesting study. For-profit hospitals more likely to admit patients from the ER vs. non-for-profit hospitals (admission can be more lucrative), which provides analytic backing for Department of Justice suits against large for-profit hospital chains...

My favorite concepts:
1)Understanding past patterns of behavior helps us change them in present & future
2)We're not aware of most of our motivations
3)Insight promotes change
4)Corrective emotional experience in therapeutic relationship
5)Taming the 'superego'

A point I continually return to is that skepticism about HOW we carve up psych problems into different conditions, illnesses and diseases is warranted, but skepticism about the existence of psych problems is absurd. And psychiatry and psychotherapy only req the latter to be legit

Residency is recognized as one of the most hostile work settings in the country. To use terms like “safe space” and “wellness” in a derogatory fashion says more about complicity in a system that destroys trainees (bodies and minds) than it does about the system itself. Do better.

Predicting Suicide Death

A recent study looking at >2M Americans presenting to ERs with MH concern shows the difficulty of 'predicting suicide' - we can't predict suicide to help guide important clinical decisions.

The problem: Sensitivity vs PPV

When referring to a specialist of any type, refer using the following phrase: 'I'd love to ask a colleague of mine with specialization in __________ for their opinion.' Not: 'you need to see _________ to get ________.'

That's what we consultants are there for!

feeling “unsure” is not a weakness

you will find people who think they
are so “sure”—tell you that it is.

it means you are still active & thinking
maybe you need more depth & meaning
in the process of arriving somewhere
rather than only final conclusions.

This week, Roger S. McIntyre, MD, FRCPC returns to explore depressive disorders. It’s a speedy, engaging, high level overview.

And fun fact: Dr. McIntyre recorded this while traveling… in a McDonald’s. Talk about commitment!
