VT Positive concordance in the precordial lead and no RBBB-like pattern in V1 but the slow activation. NW axis as well. Sinus rhythm with the typical (Strauss) LBBB.

Leave it or consult EP ❓Only at nighttime. Tiny weeny prolongation of PR interval before 2:1 AV block? Longer strips show 3:2 Wenckebach AV block (no PP prolongation to enable 1:1 conduction in M2). Bifascicular block RBBB+LAFB in 12-lead ECG. One and a half months after OMI.
