We investigated the potential solutions to improve communication between treating teams and integration of care after ICU, from the perspective of patients, caregivers, intensivists and general practitioners

We found 10 themes, across three feedback loops 🔁 ICU and primary care collaboration 🔁 developing mechanisms for improved communication across the care continuum 🔁 learning from post-ICU initiatives to improve future care

Practical solutions were identified to enhance the quality of survivorship for ICU survivors and their caregivers. Future intervention trials to develop and test these proposed solutions are required
