A 24yo male with ileal CD in clinical remission on biologic monotherapy presents for disease assessment with IUS following normal colonoscopy. IUS features suggest he needs treatment escalation or switch of medical therapy. What are the features of active inflammation?

This was reported by Ralley Prentice et al. We don't use much thiopurines in the US and I avoid combo in pregnancy unless needed for prior immunogenicity or severe disease. All IBD pts should be followed as high risk pregnancies to pick up complications pubmed.ncbi.nlm.nih.gov/38366352/

Uma Mahadevan Thanks for highlighting our work Uma Mahadevan. We use a lot of thiopurine in AUS due to prescribing laws, so monitor very closely in 🤰, split dose early in shunters, avoid dose increase & escalate to biologic early if active disease & 🛑 if abN LFTs or rising bile acids
