Our Professor Nav Kapur presented the latest findings on self-harm & #SuicidePrevention at today’s NCCMH Royal College of Psychiatrists learning set. Professor Kapur also spoke about the #SuicidePrevention strategy in 2023 & our NCISH list of 10 key elements for safer care for patients

Psych therapies are recommended by NICE for all pt who have #selfharmed, but many do not receive them. Building on our prev work (bmjopen.bmj.com/content/3/11/e…), we conducted a pt & clinician study on access to psych therapies.

(1) What did we do? We spoke to 51 liaison psych (LP) staff in 32 hosps abt barriers to accessing aftercare for ppl presenting to hosp for #selfharm, & strategies that open doors. We focussed on LP, given urgent need for aftercare following #selfharm tinyurl.com/v69s4bkn

(2)What did we find? Ultimately, significant gaps in aftercare w consequences for patient safety & further #selfharm. Access to aftercare varied across hosps. Most greatly struggled to access care, but some had examples of open & accessible services. tinyurl.com/v69s4bkn

(3) Barriers incl. PrimaryC: exclusion from IAPT for #selfharm "the misnomer of improving access"; Secondary C: thresholds so high, they had implications for patient safety; views of #selfharm as temp social crises, & not under remit of sec care. tinyurl.com/v69s4bkn

(4) others-focus on preventing inpt admis, silo working, waiting times, ➡️leading to likelihood of further #selfharm, distress & exclusion. Generally,lack of aftercare for ppl who have self-harmed, eps during immediate aftermath when risks are highest. tinyurl.com/v69s4bkn

(5) Staff wanted access to prompt aftercare for pts presenting to hosp w #selfharm, but sadly, only a minority had these services. Staff w access, greatly valued these pathways. Outpts linked w the liaison team was seen as essential for improving pt safety. What else worked?

(6) staff advocating for pts, directly challenging any assumptions/stigma around #selfharm. Not just emailing referrals, -getting on the phone, involving senior staff when personality clashes happen. Anything to get the best possible outcome for pts.

(7) what else? funding, staff (y), but also MDT input into assesmt & care plans (esp social wrkers/CPsych). ClinPsy's valuable for psych formulation &knowledge of psych therapies. Time to focus on therapeutic assessments & care plans in the ED; pos relationships in/across teams
